Assists with short and long term planning of patients, dealing with psychosocial, personal, financial and environmental concerns. Upholds the mission and values established by Memorial Healthcare. Recognizes and demonstrates understanding of patient and family centered care.PRIMARY JOB RESPONSIBILITIES * Completes patient/family assessment which includes evaluation of psychological, social, spiritual, financial, legal and environmental needs as well as bereavement risk.

  • Develops a social work plan of care with short and long term goals that have been mutually agreed upon with patient/family, in accordance with program guidelines and requirements.
  • Assists patient/family in securing community resources.
  • Provides complex social work services and psychosocial therapies to facilitate adjustment to disease and major illnesses.
  • Explores and evaluates financial situation and third party reimbursement for patient care.
  • Contacts clergy or spiritual advisor as requested by patient or family.
  • Evaluates and revises plan of care as necessary according to patient/family needs.
  • Participates in weekly team meetings for care coordination and review of the plan of care.
  • Participates in weekly Hospice Interdisciplinary group (IDG) meetings for coordination of care and updating of plan of care.
  • Maintains accurate records as required and documents all necessary information in clinical records in a timely manner.
  • Participates and assists in providing in-service education as appropriate.
  • Participates in training and orientation of new employees and volunteers.
  • Identifies own learning needs and attends educational offerings, reads educational literature as it pertains to career growth and knowledge.
  • Attends meetings, care conferences and community events as appropriate.
  • Appropriately involves and contacts other disciplines in patient care and notifies nursing staff of pertinent patient information.
  • Seeks involvement in quality improvement efforts, community organizations and service excellence.
  • Approaches patients/families in a manner that reduces conflict and prevents undue stress and anxiety.
  • Communicates effectively to patients/families regarding insurance benefits, program services and criteria requirements.
  • Communicates all pertinent information to clinical supervisors and team members as appropriate.
  • Demonstrates knowledge of and supports hospital mission, vision, and value statements, standards, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, customer service standards, and the code of ethical behavior.

DEPARTMENTAL AND ADDITIONAL JOB RESPONSIBILITIES * Possess a valid Michigan Driver’s license and reliable transportation.

JOB SPECIFICATIONSEDUCATION * Master’s degree in social work from an accredited college or university required. Graduate of an accredited school of social work is required.

  • Active LMSW license from the State of Michigan
  • BLS

EXPERIENCE * At least two years of experience in a health care facility required.

  • Clinical grief counseling experience preferred.

ESSENTIAL PHYSICAL ABILITIES/MOTOR SKILLS * Able to travel independently throughout all Memorial Healthcare facilities.

  • Ability to legally drive a car, and to travel independently throughout service area to patients’ home in all weather conditions as well as navigate independently within homes including areas confined by space and/or equipment to access a patient for the purpose of assessment and evaluation.
  • Possess small motor skills required to operate modern computer/laptop, cell phone, and other equipment utilized by Memorial Healthcare.
  • Ability to communicate assessment findings and actions by written word and document in assigned computer.

ESSENTIAL TECHNICAL ABILITIES * Proficiency using modern office, computer/laptop, and cell and telephone equipment used by Memorial Healthcare. Ability to access, create, send, and retrieve data from hospital information system.

  • Ability to document in patient’s home health/hospice electronic health record.

ESSENTIAL MENTAL ABILITIES * Ability to adapt and maintain focus in fast paced, quickly changing or stressful situations.

  • Ability to read and interpret a variety of documents including, but not limited to policies, operating instructions, white papers, regulations, rules and laws.
  • The ability to complete an assessment of patient’s social/psychological needs and develop an appropriate plan to meet the needs.
  • Ability to document assessment, interventions, and evaluation related to those needs.

ESSENTIAL SENSORY REQUIREMENTS * Able to see for the purpose of reading information received in formats including but not limited to paper, computer, reports, bulletins, updates, manuals.

  • Able to communicate clearly and effectively with patients and families.
