Job description:
The Integrated Case Manager-Resident is an integral member of the patient-centered care team, tasked with the essential responsibilities of assessment, planning, facilitation, care coordination, evaluation, and advocacy. This role is dedicated to ensuring comprehensive healthcare needs are effectively met for individuals and families through proactive communication and resource optimization, ultimately promoting patient safety, quality of care, and cost-effective outcomes.
Key Responsibilities:
– Address the needs of patients who have encountered critical events or diagnoses that necessitate intricate management strategies and significant resource utilization to enhance health outcomes throughout the care continuum.
– Deliver services to patients in both ambulatory and hospital settings.
Education & Experience Requirements:
– A Master’s degree in Social Work from an accredited program is required.
– Demonstrated ability to assess and identify barriers while collaborating with other members of the care team to address patients’ social and clinical needs.
– Proficient verbal communication and written documentation skills.
– Strong customer service and interpersonal capabilities, enabling effective interaction with all members of the patient’s care team.
– Exceptional problem-solving, analytical, and decision-making skills.

Certifications & Licensure:
– A Limited Licensed Social Worker (LLMSW) with a valid and unrestricted license from the State of Michigan is required.
