Job description:

Northwest Northland Park Home Dialysis is a specialized unit within an outpatient dialysis facility dedicated to the management of peritoneal and home dialysis patients, serving a total census of 20 peritoneal dialysis (PD) patients and 2 home hemodialysis (HHD) patients. The unit is staffed by a team of full-time Nephrologists, along with two full-time Home Nurses and on-call home dialysis technicians from a travel pool. Support staff, including a front desk team and technical leaders responsible for machine maintenance, collaborate with the outpatient clinic to assist with operational duties.

Our team case-manages 15 patients monitored by connected health technologies for Baxter and NxStage systems. Patients receive training to perform dialysis treatments at home, completed within the clinic environment—PD training typically takes 7 to 10 days, while HHD training spans 4 to 6 weeks. The home care team coordinates monthly clinic visits for all home patients alongside an interdisciplinary team, which includes conducting laboratory draws for PD patients. Patient education and referrals to transplant services are also provided, tailored to the preferences of individual patients.

The unit generally operates Monday through Friday from 8:00 AM to 4:30 PM for patient care, with flexibility to accommodate the clinic’s requirements. Home Nurses are scheduled for daily after-hours on-call duties, including weekends. The home clinic remains closed most weekends and holidays, contingent upon clinic demands such as training or clinical needs for patients.

General Summary:

  • Responsible for performing stocking and custodial duties within the assigned dialysis unit.
  • Assists with the implementation of Lean strategies to enhance operational efficiency.
  • Reports directly to the Dialysis Nurse Manager or the Technical Leader of the dialysis program, and to the charge nurse in their absence.
