Title: Clinical Nurse Navigator (RN) – Bronson Spine Surgery

The Nurse Navigator plays a pivotal role in guiding patients through the healthcare system, establishing early contact with newly diagnosed patients in the cardiovascular, stroke, orthopedic, and oncology service lines. This position requires close collaboration with primary care physicians, surgeons, specialists, ancillary services, and various healthcare facilities. Acting as a liaison, the Nurse Navigator ensures patients and their families receive comprehensive psychosocial support, access to vital resources, and education regarding disease specifics and treatment options.

In addition, this role involves working with designated physicians and teams to develop and maintain clinical pathways and assess outcomes. The Nurse Navigator is expected to uphold professionalism while consistently demonstrating and promoting the values of respect, honesty, care, and dignity for both patients and healthcare team members.

Within applicable service lines, this position coordinates care for patients participating in the Bundled Payment (BP) program. The Nurse Navigator is responsible for ongoing evaluation, quality improvement, and outcomes for BP program patients within their service line scope. Those providing direct patient care must demonstrate competencies specific to the population served.

The Breast Nurse Navigator specifically assists patients navigating the healthcare system, establishing contact with newly diagnosed Breast/Oncology patients, and collaborating closely with radiologists, primary care physicians, surgeons, specialists, and ancillary services.

Key Responsibilities:
– Engage in the assessment, planning, implementation, and evaluation of health services provided by Bronson.
– Serve as an integral support system for patients as they transition through the continuum of care, from diagnosis through treatment to survivorship.
– Maintain a Bachelor’s degree in nursing, with a minimum of 5 years of professional nursing experience in a relevant service line, and a Master’s degree preferred.
– Hold a valid Registered Nurse license in good standing with the State of Michigan; BLS certification is required within 6 months of hire, with Oncology Nursing Certification required within 3 years of hire for the Cancer Care Center.
– Demonstrate excellent interpersonal skills and the ability to solve problems and prioritize effectively while maintaining patient confidentiality.
– Communicate clearly and effectively with patients, providers, and other staff members.
– Exhibit proficiency in computer skills and the ability to interpret data from various systems, dedicating 40 to 70 percent of work time operating a personal computer or CRT.
– Maintain positive customer service and establish effective working relationships with patients, staff, and physicians within a team-oriented environment.
– Present information clearly and concisely while demonstrating effective presentation skills. Engage in patient education efforts, including leading classes and support groups.
– Negotiate skillfully in challenging situations, maintaining relationships while driving outcomes.
– Act as a liaison between patients, physicians, and community services throughout the entire patient experience, which may include post-discharge follow-up.
– Develop and maintain relationships with office staff, hospital department heads, and participating medical professionals, ensuring a seamless transition of care.
– Collaborate with providers to evaluate and implement patient care plans and provide appropriate education for patients and families regarding their care.
– Assist in accessing culturally appropriate support services, maintain accurate and confidential records, and track metrics related to assigned patients.
