Job description:
We are seeking a compassionate Licensed Practical Nurse (LPN) to become a valuable member of our expanding team. The ideal candidate will possess a positive attitude, exceptional interpersonal skills, and a current state LPN license. In this position, you will provide essential medical support to our clients while ensuring a safe and nurturing environment.
Responsibilities:
- Collaborate in the planning and coordination of comprehensive patient care alongside the Registered Nurse (RN) and the physician’s care plan, while adhering to the nursing care plan for each individual patient.
- Accurately document and communicate the patient’s condition and care, noting any signs or symptoms that may indicate changes in their health status.
- Prepare clinical progress notes and summaries in a timely manner.
- Assist patients with activities of daily living, while also educating them on appropriate self-care techniques.
- Ensure a safe environment for each patient at all times.
- Support the physician and RN in managing complex nursing situations.
- Adhere to universal precautions when delivering any form of patient care.
- Implement emergency procedures in the event of catastrophic occurrences, such as accidents, injuries, or significant changes in a patient’s condition.
Qualifications:
- A minimum of one year of experience as an LPN in a home care setting.
- Active state licensure as a Licensed Practical Nurse (LPN).
- Graduate of an accredited practical nursing program.
- Proof of current CPR certification.
- Flexibility in scheduling to meet patient needs.
- Exceptional interpersonal skills to foster productive patient relationships.
- A valid driver’s license and reliable transportation.
- Willingness to work a flexible schedule and travel locally as required.
