Position Overview:
As a Case Manager (RN) at Bronson LakeView Hospital, you will play a critical role in facilitating the patients’ journey from admission to discharge. Your responsibilities will include the assessment, planning, implementation, coordination, monitoring, and evaluation of patient care to ensure a seamless experience. You will act as a vital clinical resource for patients, families, and staff while collaborating with an interdisciplinary team to deliver a comprehensive and cost-effective continuum of care. Your oversight will contribute to the ongoing improvement of health outcomes.
Key Responsibilities:
– Conduct thorough assessments to identify patients at risk for post-hospitalization care needs.
– Develop, implement, and regularly review patient transition care plans alongside care coordination team members.
– Monitor patient progress and document according to established procedures.
– Engage with patients to provide ongoing assessment and support, conducting daily rounds on all assigned patients.
– Identify potential readmissions and collaborate with the multidisciplinary team to outline preventive interventions.
– Foster effective transitions of care between services, ensuring clarity in documentation regarding transitional planning.
– Establish and maintain strong professional relationships with physicians and nursing teams to enhance the quality of care.
– Utilize community and outpatient resources to support ongoing care needs post-discharge.
Requirements:
– Bachelor of Science in Nursing (BSN) or higher nursing degree required within eight years of hire; Master’s degree preferred.
– A minimum of three years of experience in an acute care hospital setting.
– Licensed Registered Nurse in good standing with the State of Michigan.
– Case Management Certification preferred.
– Proficient in relevant software including word processing and spreadsheet applications.
– Excellent verbal and written communication skills, with the ability to convey complex information clearly to patients and families.
– Strong critical thinking, decision-making, and problem-solving skills.
– Capacity to manage multiple priorities effectively in a fast-paced environment.
