Job description:

Unit: Second Avenue Dialysis (CA/CC & HHD)
Second Avenue Home Dialysis is a unit affiliated with an outpatient dialysis facility that supervises peritoneal and home dialysis patients, currently managing a census of 25 peritoneal dialysis (PD) patients and 5 home hemodialysis (HHD) patients. Our unit is staffed by full-time nephrologists, complemented by a dedicated team including 2 full-time Home Nurses and 1 home dialysis technician, along with various support staff shared with the outpatient clinic, such as the front desk team and tech leaders for machine maintenance, who assist with operational duties within the clinic.

We actively case manage 30 patients monitored through connected health platforms for Baxter, NxStage, and Tablo systems. Patients are trained to administer their own dialysis treatments at home, with PD training lasting 7-10 days and HHD training spanning 4-6 weeks. The home healthcare team schedules monthly clinic visits for all home patients, coordinating with an interdisciplinary team and facilitating lab draws for PD patients. Our team is also responsible for patient education and referrals to transplant services as per patient requests.

The unit typically operates Monday through Friday from 8 AM to 4:30 PM for patient care, allowing for some flexibility based on clinic needs. Home Nurses are expected to take daily after-hours on-call duties, including weekends. The home clinic is generally closed on most weekends and holidays, although this may vary depending on the requirements for patient training or clinical needs.

General Summary: Under the supervision of nursing staff, the incumbent will monitor and provide direct care to patients undergoing hemodialysis. Continuous patient monitoring and effective communication with relevant team members are essential. The role involves collaboration as part of an interdisciplinary team dedicated to the care of dialysis patients.

Education/Experience Required: A high school diploma or G.E.D. equivalent is required, with a preference for candidates possessing certification as a Nursing Assistant (NA), Medical Assistant (MA), or other healthcare-related experience. Successful completion of the Greenfield Health Systems dialysis training program is a condition for continued employment.

Certifications/Licenses Required: As a prerequisite for ongoing employment, certification as a dialysis technician must be achieved and maintained within a designated timeline of 18 months, adhering to federal regulations.
