As a Clare Bridge Program Assistant, you will assist the Clare Bridge Program Manager or Coordinator in designing and implementing a calendar of person-centered, dementia-friendly programs tailored to the unique needs of each resident within the dementia care community. You will engage residents in meaningful activities that enhance their quality of life, respecting their individual abilities, historical routines, interests, and preferences.
Key Responsibilities:
– Support the development of person-centered programs that cater to the individual needs and interests of residents.
– Assist in facilitating engagement in the seven domains of well-being—purposeful, intellectual, social, spiritual, emotional, and physical—within a dementia-friendly environment, including small sensory-focused groups for residents who may not participate in larger gatherings.
– Collaborate with the Program Manager or Coordinator in reviewing resident service plans and completing necessary documentation in accordance with Brookdale standards and state regulations.
– Ensure compliance with state and federal regulations pertaining to dementia care programs while upholding Brookdale’s mission and standards.
– Provide guidance to associates on the significance of programming and engagement for enhancing residents’ quality of life.
– Coordinate and engage in community events, including family socials and open houses.
– Communicate effectively with residents, families, and local professionals about Brookdale’s dementia care program and address residents’ personal needs and achievements.
– Assist in meeting department quality improvement initiatives, including utilizing the Brookdale Excellence Standards Tool (BEST).
– Support the planning of programming events outside of the community, which may involve driving a community motor vehicle.
– Assist with the volunteer program, including the training and supervision of volunteers.
– Perform personal care, housekeeping, and dietary responsibilities as needed to uphold service quality per Clare Bridge Program standards.

Qualifications:
– High School Diploma or equivalent is required; an Associate’s Degree is preferred.
– A minimum of one year of experience working with older adults and individuals with dementia is preferred, or an equivalent combination of education and experience.
– A valid driver’s license and a current 3-year driving abstract with no more than one moving violation are required as a condition of employment.
Physical Demands and Working Conditions:
– Frequent standing, walking, and sitting.
– Ability to reach, climb, stoop, kneel, crouch, or crawl as needed.
– Must be able to lift up to 50 pounds.
– Requires regular interaction with coworkers, residents, and vendors.
– May require occasional weekend, evening, or night shifts to cover needs.
– On-call availability as needed.
– Possible exposure to communicable diseases and infections, latex, and blood-borne pathogens.
– Occasional travel may be required.
