We are seeking a compassionate Licensed Practical Nurse (LPN) to join our expanding team. The ideal candidate will possess a proactive attitude, excellent interpersonal skills, and a current state LPN license. In this role, you will provide essential medical support to our clients while ensuring a safe and nurturing environment.

Responsibilities

  • Collaborate with the Registered Nurse (RN) and physician to plan and coordinate comprehensive patient care, adhering to each patient’s nursing care plan.
  • Accurately document and report on patients’ conditions and care, including any signs and symptoms that may indicate a change in status.
  • Prepare clinical progress notes and summaries as required.
  • Assist patients with activities of daily living, while also educating them on appropriate self-care techniques.
  • Maintain a safe and secure environment for patients at all times.
  • Support the physician and RN in complex nursing situations when needed.
  • Adhere to universal precautions during all aspects of patient care.
  • Follow emergency procedures in the event of an incident, including accidents, injuries, or any significant changes in a patient’s condition.

Qualifications

  • Minimum of one year of experience as an LPN in a home care setting.
  • Current state licensure as an LPN.
  • Graduate of an accredited practical nursing program.
  • Current CPR certification.
  • Ability to work a flexible schedule, including local travel.
  • Exceptional interpersonal skills.
  • Valid driver’s license and reliable transportation.
