At Sunrise, our Care Managers play a vital role in delivering the highest quality care and services to a dedicated group of residents and their families in our assisted living and reminiscence neighborhoods.

Responsibilities:

  • Foster meaningful relationships with assigned seniors and their families by assisting them with activities of daily living, addressing individual care needs, and understanding their unique preferences and personalities.
  • Notify management of any changes in residents’ conditions and recommend necessary adjustments to their levels of care and service.
  • Support residents in life skills and other enriching activities as outlined in their individual profiles.
  • Integrate a variety of multi-sensory experiences into residents’ daily routines.
  • Participate in the creation and monthly updates of Individualized Service Plans (ISP).
  • Be accountable for the well-being of a designated group of residents during each shift, ensuring regular physical check-ins.
  • Collaborate with the community team to ensure compliance with OSHA standards and promote safety through Risk Management programs and policies.
  • Attend to the dining needs of residents and guests, maintaining our hospitality and service standards.
  • Invite, encourage, and assist (IEA) residents in life skills and enriching activities as specified in their ISP and demographic profiles.
  • Commit to our Principles of Service in every interaction with residents and guests.

Qualifications:

  • A strong dedication and passion for serving seniors, complemented by excellent customer service skills.
  • A positive attitude, flexibility to perform various duties, and the ability to work effectively within a team environment are essential for success.
  • A high school diploma or GED is required, as per state regulations.
  • In states where applicable, maintenance of relevant certifications is necessary.
  • Candidates must be at least 18 years of age.
  • Prior experience working with seniors is preferred.
  • Possess the ability to make informed choices and act in the best interests of residents.
  • Strong written and verbal communication skills for effective interaction and understanding.
  • Proficient organizational and time management abilities.
  • Demonstrate sound judgment, problem-solving capabilities, and decision-making skills.
