As a Care Manager at Sunrise of Northville, you will play a pivotal role in enhancing the lives of our residents. Our Care Managers are dedicated to delivering exceptional care and services to a consistent group of residents and their families within our assisted living and reminiscence neighborhoods.

Key Responsibilities:

  • Cultivate meaningful relationships with a designated group of seniors and their families, providing assistance with activities of daily living while attentively addressing individual care needs and preferences.
  • Monitor changes in residents’ conditions and communicate those changes to management, recommending appropriate adjustments to care plans.
  • Facilitate life skills and other enriching activities as outlined in residents’ individual profiles.
  • Incorporate a variety of multi-sensory experiences into residents’ daily routines.
  • Contribute to the development and monthly updates of Individualized Service Plans (ISP).
  • Ensure the welfare of assigned residents by maintaining awareness of their locations and checking in with them regularly throughout your shift.
  • Collaborate with the community team to uphold OSHA compliance and promote Risk Management programs and policies, adhering to all safety regulations.
  • Attend to the dining needs of residents and guests while exemplifying Sunrise’s hospitality standards.
  • Encourage and assist residents with life skills and other enriching activities as indicated in their ISP and demographic profiles, committing to our Principles of Services.

Qualifications:

  • A strong dedication to serving seniors, coupled with excellent customer service skills.
  • A positive attitude, adaptability to fulfill various roles, and the ability to thrive in a collaborative team environment are crucial for success.
  • A high school diploma or GED is accepted, and may be required based on state regulations.
  • In applicable states, maintaining relevant certifications is essential.
  • Must be at least 18 years of age.
  • Previous experience working with seniors is preferred.
  • Ability to make informed decisions in the best interest of residents.
  • Effective written and verbal communication skills, with a sufficient level of understanding.
  • Strong organizational and time management capabilities.
  • Demonstrated judgment, problem-solving, and decision-making skills.
