Responsibilities
The Travel Hospice Aide is a compassionate, non-professional role designed to provide personal care and related services to individuals who are sick or disabled in their homes. This position operates under the supervision of a registered nurse or physical therapist, assisting patients with at least two Activities of Daily Living (ADLs). The Travel Hospice Aide or an equally qualified alternate must be available during the agency’s operating hours. Key responsibilities include:

Essential Duties and Responsibilities
During each visit, the Travel Hospice Aide:
– Measures and records vital signs, promptly notifying the primary RN of any deviations from standard practice
– Assists with at least two of the following ADLs:
– Personal hygiene (e.g., bathing, grooming, hair care, shaving, skin care)
– Nail care and oral hygiene, facilitating treatment without cutting or filing nails if the patient is diabetic
– Changing bed linens and making beds
– Supporting patients with eating and maintaining nutritional and fluid intake
– Dressing
– Assisting with ambulation, transfers, and elimination needs
– Performing incidental household services that support the patient’s health (e.g., light housekeeping, laundry)
– Aiding in prescribed exercises previously taught by a healthcare professional
– Handing medications that have been prepared for the patient
– Changing or emptying colostomy bags as directed by the RN
– Applying basic infection control principles and procedures
– Reporting any infection control concerns to the supervisor
– Following the Plan of Care as outlined by the agency’s healthcare professionals
– Documenting care in accordance with agency policies and maintaining accurate records
– Observing and reporting physical and behavioral changes in the patient’s condition to the registered nurse
– Adhering to the visit schedule and communicating any changes effectively
– Promptly contacting both patients and an Agency supervisor if unable to visit as scheduled and providing a detailed report to covering aides
– Engaging appropriately with patients, families, and agency personnel
– Attending mandatory in-services and meetings as designated by the agency, including 12 hours of in-service training per year
– Complying with agency policies and procedures
– Performing additional duties as assigned and participating in performance improvement activities
– Ensuring adherence to the 4Ms framework (What Matters, Medications, Mentation, and Mobility) to provide Age-Friendly Care

The Travel Hospice Aide is explicitly prohibited from performing the following:
– Irrigating body cavities or colostomies/wounds
– Performing gastric lavages or gavages
– Catheterizing patients
– Administering medications
– Applying heat through any method
– Caring for tracheostomy tubes
– Cutting or filing nails
– Providing personal health services not included in the patient care plan

Qualifications
Required Knowledge, Skills, and Experience:
– High school diploma or GED
– Certification as a nursing aide (CNA) with completion of a State-established training program, meeting federal guidelines
– Valid driver’s license and dependable vehicle with proof of insurance
– Ability to meet physical demands, including standing, lifting, and performing light to moderate physical tasks; ability to exert varying levels of physical effort may be required
– Experience requirements include one year of full-time direct client care in either an institutional or agency setting, or successful completion of relevant training/programs
– Willingness to undergo additional training as required by the agency and contribute to the onboarding of new hospice aides
– Attendance at staff meetings and educational in-services as mandated by the agency
– Completion of at least the 9th grade with demonstrated reading and writing ability
– Genuine interest and empathetic attitude towards caring for the sick at home
– Emotional maturity to build rapport with patients and families
– Teamwork capability in a multi-disciplinary environment
– Availability for flexible travel schedules to accommodate agency needs
– Prohibition on concurrent employment
– Independent travel ability during designated hours, with at least 80% of time dedicated to travel

Preferred Knowledge, Skills, and Experience:
– Certification as a Hospice and Palliative Nursing Assistant (CHPNA) through HPNA
