Key Responsibilities:

  • Assist clients with personal care and homemaking tasks in their homes
  • Plan and prepare meals for clients requiring dietary support
  • Provide companionship and perform light housekeeping duties
  • Follow individualized care plans to ensure optimal care under the guidance of a Registered Nurse
  • Maintain open communication with the local branch regarding client needs

Qualifications:

Responsibilities Include:

  • Provide personal care services as required
  • Deliver care under the supervision of a Registered Nurse, adhering to the established care plan
  • Assist with meal preparation and feeding as necessary
  • Perform laundry and light housekeeping duties
  • Understand and comply with infection control policies and hygiene practices
  • Report any changes in client conditions to the Registered Nurse promptly
  • Document care provided and maintain accurate client records
  • Promote a safe environment for clients by observing infection control protocols and reporting hazards
  • Demonstrate safe practices in equipment use, seeking additional training as needed
  • Communicate effectively with all interdisciplinary team members through verbal reports and participation in team meetings
  • Treat clients and their families with dignity and respect, maintaining confidentiality at all times
  • Accept and responsibly fulfill assignments from the agency
  • Engage in required in-service training to meet compliance standards
  • Perform additional related duties as required

Additional Qualifications:

  • Compliance with state training and certification requirements applicable to home health aide services
  • Current and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification, or a minimum of one year of supervised experience within the last three years
  • Minimum age of eighteen (18) years
  • Ability to read, write, and follow a written Plan of Care
  • Strong interpersonal skills and a commitment to providing compassionate care
  • Current driver’s license with a good driving record or reliable transportation
  • Preferred experience of 1+ years
