JOB OVERVIEW
The Licensed Practical Nurse (LPN) Medication Care Manager plays a critical role in delivering the highest quality of care and services by systematically administering medications and treatments in a safe and organized manner. Key responsibilities include, but are not limited to, administering medications, documenting medication administration, conducting Service and Health Updates (SHUs), and providing resident care while upholding Sunrise Senior Living’s mission: “to champion the quality of life for all seniors.” This role is performed in compliance with federal, state/provincial, and local laws, standards, and regulations, as well as Sunrise Senior Living policies, with the aim of promoting the highest standard of care for our residents.

RESPONSIBILITIES & QUALIFICATIONS
Responsibilities:

Medication Administration:

  • Deliver the highest quality of care by administering medications and treatments in a safe and organized manner.
  • Review and document pertinent resident information daily, ensuring accurate recording in the Daily Log.
  • Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
  • Administer, assist with, and monitor medications and treatments for each resident, following the medication administration record and the Six Rights of Medication Pass (“Right” resident, medication, dosage, time, route, right to refuse).
  • Ensure medications are administered at the prescribed times.
  • Document and initial as medications are administered, ensuring appropriate documentation for refusals or missed doses.
  • Maintain confidentiality of all resident information, including medication details.
  • Report any resident concerns raised during medication administration to the RCD or Wellness Nurse.
  • Restock medication carts following each medication pass.
  • Assist in verifying medication, irrespective of packaging system.
  • Assess residents to determine needs for “as needed medication”, documenting and reporting appropriately.
  • Conduct narcotic counts with another Medication Care Manager (MCM) or Lead Care Manager (LCM) at the beginning of each shift.
  • Keep the Medication Room, medication carts, and treatment carts pristine, ensuring medications are readily available and free from expiration.
  • Adhere to medication refill processes.
  • Support residents in maintaining independence while promoting dignity and physical safety, aligning with Sunrise Principles of Service.
  • Exhibit empathy and understanding, mindful of each resident’s unique communication patterns, history, and fundamental needs.
  • Practice stringent health and cleanliness standards consistently.
  • Perform audits of medication carts.

Resident Care:

  • Notify the RCD of any resident or family concerns through the LEAD process.
  • Actively participate in daily Cross Over meetings led by the LCM.
  • Conduct Service and Health Updates as instructed by the RCD.
  • Collaborate in the development of Individualized Service Plans (ISP) and Individualized Care Plans (ICP) as well as monthly updates.
  • Accurately transcribe orders.

Risk Management and General Safety:

  • Work in partnership with the community team to ensure compliance with OSHA and applicable regulations, promoting risk management programs and adhering to safety protocols.
  • Consistently practice safety procedures, including the use of Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
  • Report all accidents or incidents immediately.
  • Alert relevant parties about unsafe or hazardous conditions without delay.
  • Ensure that cords, carts, equipment, and other potential hazards are clear of exits and comply with fire safety codes.
  • Follow all infection control techniques and ensure proper placement and removal of biohazard containers.
  • Understand and apply methods for attending to and disposing of, as well as the potential exposure to, blood borne pathogens, bodily fluids, infectious materials, and hazardous materials.
  • Report any occupational exposures to blood, bodily fluids, or hazardous chemicals immediately.
  • Safely store oxygen tanks, replace empty tanks for residents, and monitor prescribed oxygen levels.

Training and Contributing to Team Success:

  • Actively engage as a team member, striving toward team objectives.
  • Uphold the Team Member Credo in daily interactions.
  • Dedicate efforts to serving residents and guests in alignment with our Principles of Service.
  • Contribute to engagement programs and processes, including participation in surveys and improvement planning workshops.
  • Attend regular meetings such as Town Hall, Department Team, Cross Over, and Medication Technician sessions, as instructed.
  • Participate in ongoing training facilitated by the RCD and neighborhood coordinators.
  • May serve as a shift supervisor and oversee other medication care managers.
  • Maintain compliance with required training and any additional training mandated by state or provincial authorities to uphold Sunrise standards consistently.

Qualifications:
To excel in this role, candidates must satisfactorily perform each essential duty. The requirements outlined in this job description reflect the necessary knowledge, skills, and abilities:

  • Valid LPN, LVN, or state/provincial licensed nurse credential.
  • In relevant states/provinces, must uphold mandatory certifications.
  • Adhere to certification and ongoing training requirements set forth by Sunrise and applicable state/provincial regulations, including:

    • Must be at least 18 years of age.
    • Preferably possess prior experience working with seniors.

    • Skill in managing multiple responsibilities.
    • Proficiency in written and verbal communication.
