Job description:

As a Home Health Aide (HHA), you will join our compassionate community dedicated to prioritizing health and well-being. Your responsibilities will include:

  • Providing direct patient care under the supervision of a Registered Nurse and following the Aide Plan of Care (POC).
  • Assisting patients with self-administered medications by opening bottles, reading labels, verifying dosages, and observing the patient as they take their medication.
  • Accurately measuring and recording patients’ vital signs, including temperature, pulse, and blood pressure, while promptly reporting any abnormalities to your supervisor.
  • Supporting patients in maintaining personal hygiene by performing or supervising bathing, grooming, skin care, shaving, oral care, nail and foot care, among other activities.
  • Assisting with patient feeding, effectively communicating basic nutrition principles, and observing and documenting food and fluid intake as needed. This includes safely positioning patients for meals and aiding in self-feeding when appropriate.
  • Supporting patients with toileting needs, including the use of bedpans or urinals and managing catheter bags and ostomy care.
  • Employing necessary skills to facilitate patient mobility, exercises, positioning, transfers, and ambulation as outlined in the Plan of Care and CenterWell Home Health policies.
  • Documenting significant changes in patient condition and care provided to ensure continuity of care, maintaining thorough and accurate records of interactions with patients, caregivers, physicians, and other healthcare professionals.
  • Practicing effective infection control measures while providing a clean, safe, and comfortable environment for all patients.
  • Assisting with light household duties, including laundry, bed changing and making, meal preparation, light housekeeping, and shopping when no other assistance is available and with an MD order.

Required Experience/Skills:

  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the past 24 months.
  • Compliance with applicable state certification requirements.
  • A valid driver’s license, auto insurance, and reliable transportation.
  • Good standing on the HHA Registry (if applicable) and completion of HHA/CNA coursework for employment with a Medicare-certified agency.
  • A minimum of one year of experience as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, or home health/hospice agency within the last 24 months.
