We are actively seeking a dedicated and compassionate Home Health Aide (HHA) to join our team on a PRN basis, providing exceptional care to individuals in their homes in North Ridgeville, OH. Job Responsibilities:

  • Provide assistance with daily tasks as outlined in the client’s personalized care plan, including feeding, meal preparation, linen changes, light housekeeping, transportation to appointments, and outdoor activities.
  • Deliver companionship and personal hygiene care in accordance with the client’s care plan, including bathing, grooming, dressing, and toileting.
  • Facilitate active and passive mobility exercises as defined by the plan of care, documenting any observed changes to the Director of Nursing.
  • Comply with agency policies, procedures, and protocols.
  • Ensure HIPAA compliance and maintain the confidentiality and security of clients’ medical information.

Job Requirements:

  • High school diploma or GED.
  • A minimum of one year of documented home care assistance and/or direct care experience in settings such as assisted living or skilled nursing facilities is preferred.
  • Completion of a certified nursing assistant or home health aide program is advantageous.
  • Valid CPR certification and a negative TB skin test or chest X-ray are required.
  • Must possess a valid driver’s license and auto insurance (proof required).
  • Physical requirements include the ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull, with the capacity to lift up to 50 pounds as necessary based on client mobility.
  • Self-directed with a capacity to work independently with minimal supervision.
  • Ability to communicate effectively in English, both in writing and verbally.
  • Up-to-date vaccinations, including COVID-19, Influenza, and Hepatitis B.
