Job description:
The Case Manager will focus on assisting single individuals and families escaping domestic violence, collaborating closely with community partners in Oakland County to identify and screen eligible households. This role will also involve landlord engagement to facilitate safe housing placements. The primary aim is to house single women and women with children in need of support.
Compensation for this position starts at $43,100 annually.
Key Responsibilities:
– Manage a caseload of 20 to 30 families, assessing individual and family needs, developing action plans, budgeting, and monitoring compliance with program requirements.
– Conduct intakes, exits, and document client meetings in the Homeless Management Information System (HMIS).
– Complete case notes and service transactions via HMIS in a timely manner, gathering necessary data to evaluate program outcomes.
– Guide clients in finding, securing, and maintaining housing.
– Conduct unit Housing Quality Standards (HQS) inspections to ensure compliance with safety and federal regulations.
– Provide a minimum of monthly in-home case management after families are housed.
– Utilize the Mainstream Benefits Access Tool to screen and connect households with appropriate benefits.
– Screen and connect households to various internal and external housing programs and resources.
– Implement and enforce program rules, policies, and procedures to maintain a safe environment.
– Maintain accurate client files and records.
– Administer and compile client assessments and evaluations.
– Make referrals to additional services as needed and collaborate with community providers to deliver coordinated services to clients.
– Record and process client program fees (rent).
– Engage with landlords for mediation, negotiation, and tenant relations.
Additional Responsibilities:
– Maintain authorization to transport clients and provide transportation services when necessary.
– Foster and coordinate inter-agency collaboration to assist clients in connecting with shelter, achieving their goals, and attaining long-term stability.
– Continuously enhance and implement case management and data collection tools.
– Uphold agency values by enforcing program rules, policies, and procedures to ensure a client-centered, inclusive environment for clients, staff, interns, and volunteers.
– Assist with front-desk operations, including answering phones and responding to crisis calls as needed.
– Participate in internal and external task forces and committees aimed at supporting the homeless population.
Minimum Qualifications:
– Lived or professional experience working with individuals experiencing homelessness or severe poverty in the U.S.
– An Associate’s degree.
– 1-2 years of experience in a role related to human/social services and/or case management.
– Reliable transportation to transport clients, with maintenance of automobile insurance.
– Valid driver’s license; Lighthouse reserves the right to require a driving record check where driving is essential to the position.
– Proficiency in Microsoft Office Suite and Google Apps.
– Flexibility to prioritize multiple tasks in a dynamic environment.
– Ability to effectively multitask in a fast-paced atmosphere.
– Experience working with diverse and challenging populations.
– Availability to work evenings and weekends.
Preferred Qualifications:
– Bachelor’s in Social Work (BSW) from an accredited college or university, or a master’s degree in a related field.
– 2+ years of experience in a role related to human/social services and/or case management.
– Understanding of social constructs affecting those facing economic instability.
– Knowledge of the social services landscape in Oakland County and neighboring regions.
– Experience with the Homeless Management Information System (HMIS).
– Capacity to self-regulate and work independently while effectively coordinating with other staff.
– A strong inclination for innovative and creative problem-solving.
– Training in Harm Reduction, Trauma-Informed Care, Housing First, and SPDAT methodologies.
– Proven ability to function effectively with racially and ethnically diverse clients and teams.
– Experience in team management and leadership.
