- Brook Park – 9 AM to 4 PM
- North Ridgeville – 12-hour Shifts – 7 AM to 7 PM, 7 PM to 7 AM
- Westlake – 10 AM to 2 PM

Job Responsibilities:
- Assist clients with daily tasks as outlined in their individualized care plans, including feeding, meal preparation, linen changes, light housekeeping, and transportation for appointments or activities.
- Provide companionship and personal hygiene care as specified in the care plan, encompassing bathing, grooming, dressing, toileting, and assistance with measurements when necessary.
- Support clients with mobility exercises defined by their care plan, documenting any observed changes to the Director of Nursing.
- Comply with agency policies, procedures, and protocols.
- Uphold HIPAA regulations, ensuring the confidentiality and security of clients’ medical information.
Job Requirements:
- High school diploma or GED.
- A minimum of one (1) year of documented home care assistance or direct care experience, preferably in assisted living or skilled nursing facilities.
- Preferred: Successful completion of a nursing assistant or certified home health aide program.
- CPR certification and a negative TB skin test or chest X-ray are required.
- Valid driver’s license and automobile insurance (proof of insurance is mandatory).
- Physical ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull, with the capacity to lift up to 50 pounds.
- Self-directed and capable of working with minimal supervision.
- Strong adherence to HIPAA and the ability to maintain the confidentiality of clients’ medical information.
- Proficiency in reading, writing, speaking, and understanding English, with effective communication skills.
- Up-to-date vaccinations, including COVID-19, Influenza, and Hepatitis B.
