Job description:

The Veteran and Assessment Case Manager is responsible for orienting 20 or more clients to the shelter each week by conducting a single adult coordinated entry service assessment, while managing a caseload of approximately 8 veteran clients simultaneously. This role operates within a collaborative team, consisting of another Assessment Case Manager and two seasonal winter intake specialists, under the supervision of the Assessment Team Manager. The primary aim of this position is to ensure a welcoming and supportive entry experience for clients at our shelter, as well as providing access to additional resources and community support services. Many clients assessed will be on a waiting list for available residential beds, and this assessment process facilitates access to all shelter services, placement on the Community Housing Prioritization list, connection to housing case management, and the opportunity to apply for housing vouchers. Furthermore, this role encompasses the provision of supportive services to veteran clients in coordination with the Healthcare for Homeless Veterans (HCHV) program. The position operates on a daytime schedule from Monday to Friday, with occasional evening and weekend commitments.

This is a salaried, exempt role with an annual salary of $48,505. General Responsibilities:

  • Assist in the orderly operation of the Delonis Center as needed.
  • Transport clients using a personal vehicle when necessary.
  • Meet with clients in the community as required.
  • Perform real-time data entry in the HMIS system to ensure case records are complete and current.
  • Provide support to fellow case managers as needed.
  • Availability to work evenings, weekends, and holidays during winter months, as assigned.
  • Undertake additional tasks as directed by the Manager.

Assessment Case Manager Responsibilities:

  • Conduct 20 or more coordinated entry service assessments for single adults experiencing homelessness on a weekly basis.
  • Understand and effectively utilize community resources to meet client needs, making appropriate referrals and determining necessary SAWC services.
  • Guide clients on accessing shelter services and obtaining a housing case manager and/or a Housing Choice Voucher.
  • Maintain a Triage list for clients requiring a residential bed, including follow-up for those who have been on the list for over a month.
  • Complete Housing Choice Voucher pre-applications and verification letters for clients unassisted in assessments.
  • Connect clients with SAWC’s Client Resource and Diversion Specialist when diversion from shelter is feasible.
  • Provide Front Desk Coverage and monitor the Community Meal dining room as necessary.
  • Engage actively in case review team meetings and community housing coordination meetings.

Veteran Case Management Responsibilities:

  • Facilitate initial assessments/intake for veteran clients entering the shelter from residential and winter programs.
  • Support veteran clients with necessary services and housing case management.
  • Communicate referrals for veterans to claim Community Emergency Residential services (CERs) at the Delonis Center.
  • Ensure a welcoming orientation and comfortable shelter experience for each veteran utilizing one of the five CER beds.
  • Use SQUAREs to determine eligibility for veteran client services.
  • Refer eligible veteran clients to SSVF housing assistance programs.
  • Timely complete all documentation required by the Veteran Administration.
  • Conduct real-time data entry in the HMIS system to maintain thorough and current client information, including case notes for veteran clients.
  • Attend community-level Veteran Community Housing Prioritization meetings.
  • Collaborate with the Veteran Outreach Case Manager, Peer Support Specialist, and Contracting Officer to address veteran client needs and develop individual service plans.

Qualifications:

  • A Bachelor’s degree in Social Work or a related field is required; a Master’s in Social Work or a similar degree is preferred. Candidates with an LLMSW must commit to completing LMSW status by an agreed-upon date. Equivalent relevant experience will also be considered.
  • 1-2 years of experience working within a healthcare or human services organization is preferred.
  • Strong diagnostic skills, particularly in relation to mental health and substance abuse issues.
  • Familiarity with HMIS or similar electronic record-keeping systems is highly desirable.
  • Knowledge of the healthcare system and local community resources.
  • Excellent verbal and written communication skills are essential.
  • Strong critical thinking abilities.
