Compensation will be commensurate with the applicant’s skills and experience, and the role is also eligible for team-based bonus opportunities. Please note that temporary associates do not qualify for benefits but may participate in the company’s 401(k) program.

Responsibilities:
The Clare Bridge Program Assistant supports the Clare Bridge Program Manager or Coordinator in developing and implementing a calendar of person-centered, dementia-friendly programs tailored to the unique needs of residents within our dementia care community. Aligning with Brookdale’s Resident and Family Engagement philosophy, you will provide meaningful and purposeful care programs that enhance the quality of life for residents, placing value on their individual abilities, habits, historical routines, interests, and preferences.

Essential Functions:

  • Assist in implementing a person-centered program that caters to the individual needs and interests of residents, engaging them in seven domains of well-being—purposeful, intellectual, social, spiritual, emotional, and physical—in a structured, dementia-friendly environment, which includes small sensory-focused groups for those unable to participate in larger gatherings.
  • Collaborate with the Program Manager or Coordinator to review individual resident service plans and complete program documentation as required by Brookdale standards and state regulations.
  • Ensure compliance with state, federal, and other regulations surrounding dementia care programs, upholding Brookdale’s mission, policies, procedures, and standards.
  • Offer guidance and resources to associates regarding programming and engagement that enhances the quality of life for residents.
  • Participate in and coordinate special events at the community, including family socials, marketing/sales initiatives, and open houses.
  • Communicate effectively and positively with residents, families, and community professionals about Brookdale’s dementia care program and its benefits, while also addressing residents’ personal needs and accomplishments.
  • Assist the Manager or Coordinator in achieving departmental quality improvement initiatives, including the Brookdale Excellence Standards Tool (BEST).
  • Support plans and programming events outside the community, which may involve driving a community vehicle.
  • Assist with volunteer programs, including training and supervising volunteers as needed.
  • Perform personal care, housekeeping, and dietary responsibilities to maintain service quality in accordance with Clare Bridge Program standards.

Qualifications:

  • High School Diploma or equivalent is required; an Associate’s Degree is preferred.
  • A minimum of one year of experience working with older adults and individuals with dementia is preferred, or equivalent education and experience.

Certifications, Licenses, and Other Special Requirements:

  • A valid driver’s license and a current three-year driving abstract with no more than one moving violation are required as a condition of employment.

Physical Demands and Working Conditions:

  • Continuous standing, walking, and sitting.
  • Ability to use hands and fingers for handling tasks.
  • Requires reaching, climbing, balancing, stooping, kneeling, or crawling as necessary.
  • Effective communication skills are necessary, including the ability to listen.
  • Must be able to lift up to 50 pounds.
  • May require interaction with co-workers, residents, or vendors.
  • Some weekend, evening, or night shifts may be necessary for coverage.
  • On-call status may be required.
  • Potential exposure to communicable diseases, latex, blood-borne pathogens, as well as various drugs, chemicals, infections, or biological hazards.
  • Occasional travel may be needed.
