Base pay will be determined by the applicant’s skills and experience, and the position is also eligible for team-based bonus opportunities. Responsibilities:
The role involves providing dementia care that includes tailored activities of daily living (ADLs), meaningful engagement, and effective communication with residents, adhering to individualized service plans. You will treat residents with respect and dignity, recognizing their unique needs and encouraging their independence while fostering an engaging community culture. Duties include:
- Assisting residents with their care plans and daily living activities such as bathing, dressing, grooming, toileting, and transferring.
- Participating in behavioral expression meetings and discussions with community leadership.
- Understanding each resident’s life story, preferences, and historical routines.
- Serving meals in the dining room, preparing meals according to dementia-friendly menus, and promoting healthy eating habits while respecting residents’ choices. Documenting and reporting any changes in eating habits to supervisors.
- Initiating and leading at least one Daily Path engagement program, encouraging socialization and participation in planned activities.
- Maintaining accurate daily records in compliance with company policies, including documentation related to medication distribution, resident engagement interests, and any observed changes in residents’ physical or emotional conditions.
- Assisting residents with medication as permitted by state regulations, in accordance to physician orders and established medication policies.
- Keeping a clean, safe, and orderly environment for residents through general housekeeping duties.
- Responding promptly to emergencies and resident requests for assistance.
- Monitoring resident locations and promptly reporting any concerns to supervisors.
- Promoting teamwork through positive interactions with colleagues.

Qualifications:
- Education: High school diploma or GED is preferred, along with three to six months of related experience or training in direct service with older adults living with dementia, or an equivalent combination of education and experience. - Knowledge of dementia, specifically Alzheimer’s disease, is essential.
Certifications, Licenses, and Special Requirements:
- Current state certification may be required in accordance with local laws, with adherence to regulations for maintaining certification.
Physical Demands and Working Conditions:
- Standing, walking, sitting, and using hands and fingers for various tasks.
- Ability to lift up to 50 pounds.

- On-call duties may be required as needed.
