InterCare is currently seeking a Full-Time MAT Clinical Support Nurse, with flexible scheduling opportunities, to serve across our Holland, Pullman, and Bangor locations. Work Schedule:
Typically, hours are Monday to Friday from 8:00 a.m. to 5:00 p.m., with occasional late patient appointments. Schedules may vary based on business needs, with flexible shifts available. There are no weekend or major holiday commitments.

Minimum Requirements:
– A thorough understanding of Substance Use Disorder (SUD) programming and Medication-Assisted Treatment (MAT) management derived from completion of a SUD training program, a Registered Nursing (RN) degree, and practical experience in a professional healthcare setting.
– A current RN license in the State of Michigan.

Primary Accountability:
As a member of the clinical care team, the MAT Clinical Support Nurse will provide high-quality, holistic nursing care to patients dealing with Substance Use Disorder (SUD) through a client-centered approach. Responsibilities include collaborating with team members to develop and implement SUD programming across all sites, assisting patients in achieving their recovery goals.

Description of Primary Duties & Responsibilities:

1) Develop individualized patient care plans:
– Conduct or oversee a comprehensive patient assessment encompassing medical, functional, cognitive, affective, psychosocial, nutritional, and environmental aspects.
– Collaborate with Behavioral Health Providers (BHP) and Primary Care Providers (PCP) to formulate and execute individualized care plans in accordance with the American Society of Addiction Medicine (ASAM) Criteria.
– Initiate evidence-based care planning with patients and families.
– Aid patients in establishing realistic self-management goals and implementing care plans, while identifying barriers to achieving these goals.
– Monitor patient progress against treatment and self-management goals.
– Regularly review and update care plans in conjunction with the BHP, peer recovery coach, and patient to evaluate success.
– Implement systematic processes for patient intake, education, and ongoing follow-up.
– Support site staff in providing patient education and relapse prevention assistance.
– Ensure ongoing monitoring of patient compliance with follow-up and tracking.
– Facilitate interdisciplinary team huddles to discuss and evaluate SUD programming clients.
– Maintain a strong proficiency in eCW and collaborate with the care team to ensure accurate documentation.

2) Oversight of health centers to ensure compliance with Vivitrol stock storage and handling requirements, along with providing recommendations to meet such standards.
– Ensure that all sites maintain an effective emergency response plan for power outages.
– Provide inventory support to sites as necessary.
– Assist site staff with Vivitrol administration when required.

3) Foster community relationships:
– Aid in establishing referral and coordination agreements.
– Participate in outreach initiatives to address additional community needs.
– Serve as a liaison to community partners, including mental health organizations, judicial systems, substance abuse treatment facilities, and other relevant agencies.
