Job description:

The Shelter Association of Washtenaw County (SAWC) is seeking a dedicated Assessment Case Manager to join our team. Reporting directly to the Assessment Team Manager and working closely with the Program Director, this position is integral to our mission of ending homelessness, one person at a time.

About SAWC:
With over eight million Americans teetering on the brink of homelessness, and approximately 5,000 individuals currently experiencing homelessness in Washtenaw County, SAWC has been at the forefront of providing crucial services and emergency shelter since our establishment in 1982. Housed at the Robert J. Delonis Center in downtown Ann Arbor, we serve a wide array of individuals, including those facing first-time homelessness, veterans, survivors of domestic violence, and people dealing with substance abuse or mental health challenges. Our core programs include Residential Emergency Shelter, Non-Residential Support, Shelter Diversion, Critical-Time Intervention, and Warming Shelter, collectively assisting nearly 1,500 clients each year.

Position Profile:
The Assessment Case Manager will be responsible for orienting 20 or more clients to the shelter each week by conducting coordinated entry service assessments for single adults experiencing homelessness. This role involves collaboration with another Assessment Case Manager and two seasonal winter intake specialists, all under the guidance of an Assessment Team Manager. The primary objective is to ensure a welcoming and supportive entry experience while providing clients with additional resources and referrals to community support services. Clients completing assessments may need to wait for available residential beds, but by completing the assessment, they gain access to all shelter services, are placed on the Community Health Prioritization list, connect with housing case management, and have opportunities to enroll in housing voucher programs. The position requires availability from 11:30 AM to 8:00 PM, with potential for weekend adjustments.

This is a salaried, exempt position with an annual salary of $47,049. Duties and Responsibilities:

General Responsibilities:

  • Assist in the orderly operation of the Delonis Center.
  • Transport clients as needed using a personal vehicle.
  • Meet clients in the community as required.
  • Perform real-time data entries in the HMIS system to maintain accurate and updated case records.
  • Provide support to fellow case managers as necessary.
  • Be available for weekend and evening shifts during winter months and holidays, as assigned.
  • Complete other assignments as directed by the manager.

Assessment Case Manager Responsibilities:

  • Conduct 20 or more coordinated entry service assessments weekly for single adults experiencing homelessness.
  • Understand and leverage community resources to meet client needs, providing appropriate referrals and guidance on SAWC services.
  • Educate clients on accessing shelter services and obtaining housing case management or a Housing Choice Voucher.
  • Maintain and update a Triage list for clients in need of a residential bed, including follow-ups with clients who have remained on the list for over a month.
  • Complete Housing Choice Voucher pre-applications and homeless verification letters for eligible clients as per community and MSHDA guidelines.
  • Serve as a liaison with community partners and MSHDA regarding Housing Choice Voucher matters.
  • Connect clients with the Client Resource and Diversion Specialist when shelter diversion is feasible.
  • Assist with front desk coverage and community meal monitoring as needed.
  • Attend and contribute to case review team meetings and community housing coordination meetings.

Qualifications:

  • Bachelor’s degree in Social Work or a related field is required; a Master’s degree in Social Work or a similar discipline is preferred. Candidates with LLMSW must commit to obtaining LMSW status by an agreed-upon date. Related experience may be considered in lieu of formal education.
  • 1-2 years of experience in health care or human services organizations is preferred.
  • Strong diagnostic skills, particularly related to mental health and substance abuse issues.
  • Familiarity with HMIS or similar electronic record systems is strongly preferred.
  • Knowledge of the healthcare system and available community resources.
  • Excellent verbal and written communication skills.
  • Strong critical thinking abilities.
  • A positive attitude, adaptability to change, and a genuine commitment to our mission of ending homelessness.
  • Proficiency in Microsoft Office, including Word, Excel, Outlook, and Access.
  • Exceptional organizational skills.
  • Willingness to adhere to the NASW Code of Ethics.
