Position: Home Health Aide (HHA)

As a dedicated Home Health Aide, you will play a pivotal role in providing compassionate care to our patients, under the direction of a Registered Nurse (RN) and in accordance with the individualized Aide Plan of Care (POC). Your responsibilities will include, but are not limited to, the following:

  • Deliver direct patient care in alignment with established care plans.
  • Assist patients with self-administered medications by ensuring medication safety, including opening medication containers, reading labels, verifying dosages, and observing the patient during administration.
  • Accurately monitor and document vital signs, such as temperature, pulse, and blood pressure, and promptly report any anomalies to your supervisor.
  • Support patients in maintaining personal hygiene through direct assistance or supervision with bathing, grooming, skincare, shaving, oral care, nail and foot care, and related activities.
  • Aid individuals in their nutritional needs, including meal positioning and assistance with feeding or self-feeding, while monitoring and documenting food and fluid intake.
  • Assist with toileting needs, including the use of bedpans and catheters, while following established protocols for ostomies.
  • Provide assistance with patient mobility, exercises, positioning, transfers, and ambulation in compliance with the Plan of Care and CenterWell Home Health policies.
  • Record and report changes in patient condition and document relevant interactions with patients, caregivers, physicians, and fellow staff members comprehensively and legibly, within the designated timeframe.
  • Adhere to infection control principles to maintain a clean, safe, and comfortable environment.
  • Willingly support additional household tasks, including light laundry, changing bedding, meal preparation, housekeeping, and shopping, when necessary and appropriately authorized.

Utilize Your Skills to Make a Difference

Required Experience/Skills:

  • High school diploma or equivalent.
  • Completion of a Certified Nursing Assistant (CNA) or Certified Home Health Aide program within the past 24 months.
  • Compliance with applicable state certification requirements.
  • A valid driver’s license, auto insurance, and reliable transportation.
  • Good standing on the HHA Registry (if applicable) and successful completion of the HHA/CNA course for employment with a Medicare-certified agency.
  • A minimum of one year of experience as a Home Health Aide or Certified Nursing Assistant within the last 24 months in a hospital, nursing home, or home health/hospice setting.

Scheduled Weekly Hours: 1
