Job description:
We are seeking a dedicated Licensed Practical Nurse (LPN) to become a valued member of our expanding team. The ideal candidate will possess a proactive attitude, exceptional interpersonal skills, and a current state LPN license. In this position, you will provide essential medical support to our clients while ensuring a safe and nurturing environment.
Responsibilities:
- Collaborate in the planning and coordination of comprehensive patient care in alignment with the Registered Nurse and the physician’s care plan, adhering to the individual nursing care plan for each patient.
- Accurately document and report the patient’s condition and care, highlighting any signs and symptoms that may indicate changes in health status.
- Prepare and maintain clinical progress notes and summaries.
- Assist patients with daily activities while educating them on appropriate self-care techniques.
- Create and uphold a safe environment for the patient.
- Support the physician and Registered Nurse in complex nursing scenarios.
- Adhere to universal precautions during all aspects of patient care.
- Implement emergency procedures as necessary in response to catastrophic events, incidents, or significant changes in the patient’s condition.
Qualifications:
- Minimum of one year of experience as an LPN in a home care setting.
- Hold a current state license as an LPN.
- Graduated from an accredited practical nursing program.
- Proof of current CPR certification.
- Flexibility in working hours and the ability to travel locally.
- Excellent interpersonal and communication skills.
- Valid driver’s license and reliable transportation.
