Job description:
We are seeking a compassionate Licensed Practical Nurse (LPN) to become an integral part of our expanding team. The ideal candidate will possess a positive attitude, exceptional interpersonal skills, and a valid state LPN license. In this position, you will provide vital medical support to our clients while ensuring their safety and well-being.
Responsibilities:
- Collaborate with the Registered Nurse (RN) and physician to plan and coordinate comprehensive patient care, adhering to the established nursing care plan for each individual.
- Accurately document and report on the patient’s condition and care, including observations of any signs and symptoms indicative of changes in health status.
- Prepare detailed clinical and progress notes, along with summaries as required.
- Assist patients with activities of daily living while teaching effective self-care techniques.
- Ensure a safe environment for all patients at all times.
- Support the physician and Registered Nurse in managing complex nursing situations.
- Adhere to universal precautions during all aspects of patient care.
- Implement emergency procedures in the event of any catastrophic incidents, accidents, injuries, or significant changes in the patient’s condition.
Qualifications:
- A minimum of 1 year of experience as an LPN in a home care setting.
- Current state licensure as an LPN.
- Graduate of an accredited practical nursing program.
- Proof of current CPR certification.
- Capability to work a flexible schedule.
- Strong interpersonal skills.
- Valid driver’s license and access to reliable transportation.
- Willingness to travel locally as needed.
