Job description:

We are seeking a compassionate Licensed Practical Nurse (LPN) to become an integral part of our expanding team. The ideal candidate will possess a positive attitude, exceptional interpersonal skills, and a valid state LPN license. In this position, you will provide vital medical support to our clients while ensuring their safety and well-being.

Responsibilities:

  • Collaborate with the Registered Nurse (RN) and physician to plan and coordinate comprehensive patient care, adhering to the established nursing care plan for each individual.
  • Accurately document and report on the patient’s condition and care, including observations of any signs and symptoms indicative of changes in health status.
  • Prepare detailed clinical and progress notes, along with summaries as required.
  • Assist patients with activities of daily living while teaching effective self-care techniques.
  • Ensure a safe environment for all patients at all times.
  • Support the physician and Registered Nurse in managing complex nursing situations.
  • Adhere to universal precautions during all aspects of patient care.
  • Implement emergency procedures in the event of any catastrophic incidents, accidents, injuries, or significant changes in the patient’s condition.

Qualifications:

  • A minimum of 1 year of experience as an LPN in a home care setting.
  • Current state licensure as an LPN.
  • Graduate of an accredited practical nursing program.
  • Proof of current CPR certification.
  • Capability to work a flexible schedule.
  • Strong interpersonal skills.
  • Valid driver’s license and access to reliable transportation.
  • Willingness to travel locally as needed.
