Description:
Hours/Shift: 4:00 p.m. – 12:00 a.m. EST / 1:00 p.m. – 9:00 p.m. PST, with a rotation every third weekend.

We are seeking a skilled and compassionate Virtual Care Center RN Coordinator to join our team, dedicated to delivering exceptional care to patients in a virtual environment. This role involves monitoring, assessing, and coordinating patient care using telehealth technology. The RN Coordinator will collaborate with patients, families, and the healthcare team to ensure that virtual care is provided safely, efficiently, and effectively. This dynamic position blends clinical expertise with telecommunication skills, offering patients ongoing support from the comfort of their homes.

What You Will Do:

  • Prioritize virtual care and telemonitoring alerts efficiently based on patient conditions, physician orders, and available resources.
  • Conduct thorough assessments of clients upon admission to the virtual care center, at designated time points, and as needed, utilizing virtual care equipment.
  • Educate clients and caregivers about the virtual care center, available services, financial obligations (if applicable), and their rights and responsibilities before initiating care.
  • Employ interviewing and evaluation techniques to assess clients while applying nursing judgment consistent with practice standards to formulate interventions and recommendations for physicians, agencies, and clients.
  • Report any changes in client conditions promptly to the respective physician and/or Case Manager/designee and obtain necessary orders for adjustments to the treatment plan.
  • Initiate and ensure the implementation of medication and treatment orders as prescribed by the physician in accordance with the established treatment plan.

Minimum Qualifications:

  • Bachelor of Science in Nursing (BSN) from an accredited nursing program is required.
  • A valid Compact RN License is mandatory.
  • A minimum of two to three years of experience in home healthcare or equivalent acute care settings such as cardiac, ICU, CCU, ER, triage med-surg, or home health telemonitoring.
  • Hospice and/or home care experience is preferred.
  • Familiarity with Epic or Home Care EMR is advantageous.
  • Strong clinical, dietary, pharmacology, chronic disease management, and customer service skills are essential.
  • Proven ability to achieve hospital readmission rates below 5%.
  • Excellent communication and interpersonal skills to facilitate effective interactions with healthcare professionals, patients, and other relevant parties.
  • Demonstrated proficiency in health coaching and motivational interviewing techniques to support patients in reaching their health goals; Health Coach Certification is a plus.
