Job description:

St. Mary Home Dialysis is a specialized unit within an outpatient dialysis facility, dedicated to managing peritoneal and home dialysis patients. The unit currently serves a patient census of 16 peritoneal dialysis (PD) patients and 1 home hemodialysis (HHD) patient. Our team is comprised of full-time nephrologists, along with part-time home nurses and a travel pool home dialysis technician. Additionally, we benefit from the support of shared staff members from the outpatient clinic, including a front desk team and technical leaders responsible for machine maintenance and operational duties.

We manage the care of 17 patients utilizing connected health platforms from Baxter and NxStage systems. Patients receive training to perform dialysis treatments at home within our clinic; PD training typically spans 7-10 days, while HHD training lasts 4-6 weeks. The home care team coordinates monthly clinic visits for all home patients in collaboration with the interdisciplinary team, which includes laboratory draws for PD patients. Our team is committed to providing patient education and facilitating referrals to transplant services as requested by patients.

The unit generally operates Monday through Friday from 8:00 AM to 4:30 PM for patient care, with some flexibility based on clinic requirements. Home nurses are scheduled for daily after-hours on-call duties, including weekends. The home clinic observes closure during most weekends and holidays, contingent upon clinic needs related to training or patient care.

Responsibilities include performing stocking and custodial tasks at the assigned dialysis unit and assisting with the implementation of Lean strategies. The position reports to the Dialysis Nurse Manager or the Technical Leader of the dialysis program, as well as the charge nurse in their absence.

Qualifications – External

Education & Experience: A high school diploma or GED is required.
