As a Hospice Aide/Certified Nursing Assistant, your responsibilities will include:

– Reporting directly to the Administrator, Executive Director, or Senior Patient Care Manager and functioning under the supervision of a Registered Nurse or Clinical Supervisor.
– Assisting patients in maintaining good personal hygiene and a safe, healthy living environment.
– Preparing and serving light meals, as well as assisting with patient feeding when necessary.
– Supporting patient ambulation and safely guiding the use of walkers or wheelchairs, as instructed by nursing staff.
– Performing light housekeeping duties, including removing trash and changing linens.
– Assisting with treatments as per the aide care plan and applicable state regulations.
– Engaging patients in activities to promote mental alertness and providing emotional and psychological support to both patients and their families.
– Reporting any changes in patients’ mental or physical conditions to the nursing team and participating in interdisciplinary meetings.
– Offering fundamental care practices to primary caregivers and adhering to infection control procedures.
– Documenting visit itineraries accurately and attending staff meetings.
– Participating in at least one Quality Improvement initiative annually, as assigned by supervision and completing required documentation punctually.

We are seeking individuals with the following qualifications:

– A compassionate heart, displaying empathy and kindness towards patients and their families.
– The ability to provide care in various environments, meeting patients where they are most comfortable.
– A flexible demeanor and the ability to remain composed during crisis situations.
– A valid Certified Nursing Assistant (CNA) license.
– A minimum of six months of experience in patient care, with a preference for backgrounds in hospice, home health, ICU, geriatric, or oncology settings.
– Preferable additional experience which may enhance success in this role includes hospital settings (Staff RN, Med Surg, PACU, Float Pool, ER) and critical care or acute care.
– A current driver’s license and automobile insurance.
– Current CPR certification.
