Job description:
Place of Employment: Family & Friends Homecare, LLP and Residents of FFHC assigned clients

Accountability: This position operates under the guidance of the FFHC Health Care Specialist (LPN) and the Sr. Health Care Specialist (RN).

Summary: The role of the caregiver involves providing essential daily living and personal care services within the homes of assigned FFHC patients, in accordance with a health assessment plan that reflects the approved physician-ordered service plan.

Work Hours: This position offers both full-time and part-time opportunities, requiring flexibility in scheduling to accommodate various fixed and flexible shift hours during weekdays and weekends, based on client needs.

Duties and Responsibilities:
– Deliver healthcare services to patients and residents.
– Assist clients with personal care tasks, including personal hygiene.
– Maintain meticulous patient documentation, including date, time, task, care, and service provided.
– Facilitate open communication between clients’ families and FFHC care staff (including LPNs, RNs, STNAs, and the Chief Nurse) concerning any medical or emotional issues.
– Assist patients with scheduling appointments (doctors, transportation) as well as providing help with housekeeping, laundry, and shopping services.
– Ensure the highest standards of client safety and well-being.
– Exhibit excellent written and verbal communication skills.
– Participate in completing Performance Evaluation Plans and Annual Evaluations, and engage with supervisors as directed.
– Uphold confidentiality, integrity, quality care, and professionalism in all aspects of work.

Qualifications:
– Ability to pass caregiver certification and skills lab training.
– Current CPR/First Aid certification and compliance with mandatory continuing education, maintaining 75 hours of CEUs every two years.
– Must be physically and mentally capable of managing the demands of caregiving.
– Successful completion of background checks (BCI and FBI) and drug screening.
