At Sunrise, the Care Manager plays a pivotal role in delivering the highest quality of care and services to a dedicated group of residents and their families within our assisted living and reminiscence neighborhoods.
In this position, you will:
– Foster meaningful relationships with residents and their family members while assisting them with activities of daily living, addressing individual care needs, and understanding their unique preferences and personalities.
– Communicate any changes in residents’ conditions to management, recommending appropriate adjustments to their level of care and service.
– Support residents in acquiring life skills and engaging in enriching activities as outlined in their individual profiles.
– Integrate a variety of multi-sensory experiences into the daily routines of residents.
– Contribute to the development and monthly updates of Individualized Service Plans (ISP).
– Be responsible for a designated group of residents during your shift, ensuring their well-being and physically checking on them throughout the day.
– Collaborate with the community team to uphold OSHA compliance and promote Risk Management programs and policies, ensuring adherence to safety regulations.
– Attend to residents’ dining needs while maintaining Sunrise’s hospitality and service standards.
– Invite, encourage, and assist residents in life skills and other enriching activities as indicated on their ISP and demographic profiles.
– Uphold our Principles of Service through a commitment to serving residents and guests.
Qualifications for this role include:
– A strong dedication and passion for serving seniors, coupled with excellent customer service skills.
– A positive attitude, flexibility to undertake a variety of duties in service to residents, and the capacity to work effectively within a team environment.
– A high school diploma or GED, as required by state regulations.
– In relevant states, maintenance of appropriate certifications is necessary.
– Candidates must be at least 18 years of age.
– Previous experience working with seniors is preferred.
– The ability to make informed choices and act in the best interests of residents.
– Proficiency in written and verbal communication skills, along with a solid level of understanding.
– Strong organizational and time management skills.
– Demonstrated judgment, problem-solving capabilities, and decision-making skills.
