Job Overview
At Sunrise, our Care Manager plays a vital role in enhancing the quality of life for our residents in assisted living and reminiscence neighborhoods. Responsibilities:

  • Establish meaningful relationships with a designated group of seniors and their families, assisting with activities of daily living, addressing individual care needs, and understanding their unique preferences and personalities.
  • Communicate any changes in residents’ conditions to management and suggest necessary adjustments in care and services.
  • Support residents in life skills and enriching activities as outlined in their individual profiles.
  • Integrate various multi-sensory experiences into residents’ daily routines.
  • Engage in the development and monthly updates of Individualized Service Plans (ISP).
  • Maintain oversight of designated residents during each shift, ensuring their safety and well-being through regular check-ins.
  • Collaborate with community teams to uphold OSHA compliance and promote risk management policies, while adhering to safety regulations.
  • Attend to dining needs for residents and guests, upholding Sunrise’s standards of hospitality and service.
  • Employ our Invite, Encourage, Assist (IEA) approach to support residents in skill development and other enriching activities as indicated in their ISP.
  • Demonstrate a dedicated commitment to service, guided by our Principles of Services.

Qualifications:

  • A strong dedication to serving seniors with exceptional customer service skills.
  • A positive attitude, flexibility in performing various duties, and the ability to thrive in a team-oriented environment.
  • A high school diploma or GED is preferred and may be required under state regulations.
  • Relevant certifications must be maintained where state laws necessitate.
  • Candidates must be at least 18 years of age.
  • Previous experience working with seniors is highly desirable.
