At Sunrise of Shelby, the Care Manager plays a pivotal role in delivering the highest standards of care and services to a consistent group of residents and their families within our assisted living and reminiscence neighborhoods.

In this position, you will have the opportunity to build meaningful relationships with seniors and their families, providing essential assistance with activities of daily living while attending to individual care needs and understanding their unique preferences. Your responsibilities will include monitoring changes in residents’ conditions and recommending adjustments to their care plans, as well as facilitating life-enriching activities tailored to each resident’s profile.

You will also take part in the development and monthly updating of Individualized Service Plans (ISP), ensuring that you are actively engaged with your designated group of residents throughout your shifts. Working collaboratively with the community team, you will help uphold compliance with OSHA standards and promote risk management protocols while adhering to safety regulations. Additionally, you will respond to the dining needs of residents and guests, exemplifying Sunrise’s commitment to hospitality and service.

Ideal candidates will demonstrate a passion for serving seniors with outstanding customer service skills, a positive attitude, and the flexibility to perform various duties in a team-oriented environment. A High School diploma or GED is required, as well as a dedication to maintaining appropriate certifications where applicable. Candidates must be at least 18 years of age and previous experience in senior care is preferred. Strong communication skills, effective organizational abilities, and sound judgment are essential for success in this role.
