At Sunrise of Shelby, the Care Manager plays a crucial role in delivering the highest standard of quality care and services to a dedicated group of residents and their families within our assisted living and reminiscence neighborhoods.

Responsibilities:

  • Foster meaningful relationships with assigned seniors and their families by providing assistance with activities of daily living, addressing individual care needs, and understanding each resident’s unique preferences and personalities.
  • Monitor and report any changes in residents’ conditions to management, while recommending adjustments to their level of care and service.
  • Support residents in life skills and engage them in life-enriching activities as outlined in their personalized profiles.
  • Integrate a variety of multi-sensory experiences into residents’ daily routines.
  • Contribute to the development and updating of Individualized Service Plans (ISP).
  • Take responsibility for the designated group of residents during the shift, ensuring physical checks and awareness of their whereabouts.
  • Collaborate with the community team to maintain compliance with OSHA requirements and promote Risk Management programs and policies, adhering to all safety rules and regulations.
  • Address the dining needs of residents and guests while upholding Sunrise’s hospitality and service standards.
  • Invite, encourage, and assist residents in life skills and activities as indicated in their ISP and demographic profile.
  • Uphold a commitment to serving our residents and guests according to our Principles of Services.

Qualifications:

  • A genuine dedication to serving seniors, coupled with exceptional customer service skills.
  • A positive attitude, flexibility to perform various duties in service to residents, and the ability to thrive in a team-oriented environment.
  • A high school diploma or GED is accepted and may be required based on state regulations.
  • Maintenance of appropriate certifications as required by state laws.
  • Applicants must be at least 18 years of age.
  • Previous experience working with seniors is preferred.
  • Ability to make informed decisions and act in the best interest of residents.
  • Proficient verbal and written communication skills.
  • Strong organizational and time management capabilities.
  • Demonstrated good judgment, problem-solving abilities, and decision-making skills.
