Job description:
The Case Manager plays a vital role as a member of the patient-centered care team within the hospital or specialty-based environment. This position entails the collaborative efforts of identifying, assessing, planning, facilitating, coordinating care, evaluating, and advocating for options and services that address the healthcare needs of individuals and their families. The primary objectives of the Case Manager include enhancing patient safety, ensuring quality care, and achieving cost-effective outcomes. Additionally, the Case Manager is responsible for providing effective discharge planning services that facilitate a safe transition to the subsequent level of care and address the needs of patients who have encountered critical events or diagnoses requiring management strategies to optimize health outcomes across the care continuum.
Education and Experience Requirements:
Candidates must possess a Bachelor’s degree in Nursing or a Master’s degree in Social Work, along with a minimum of three (3) years of clinical experience. Outstanding verbal communication and written documentation skills are essential, as are exceptional customer service and interpersonal skills that enable interaction with internal and external clients at all organizational levels. Candidates should demonstrate strong problem-solving, analytical, and decision-making capabilities, as well as proficient computer skills. Experience in discharge planning, home health care, rehabilitative medicine, or managed care is preferred, along with familiarity with preventive service guidelines, clinical practice guidelines, behavior change theory, and regulations pertaining to Medicare and Medicaid. Knowledge of medical ethics and legal considerations in case management, as well as an understanding of social determinants of health and their impact on patient well-being, is also required. The ideal candidate will be adept at facilitating access to community resources catering to diverse populations and possess robust organizational, planning, and implementation skills, allowing them to manage the needs of multiple complex patients concurrently. A strong sense of compassion and a commitment to advocating for patients and their families are indispensable attributes for this role.
Certifications and Licensures Required:
A valid, unrestricted Registered Nurse (RN) license or Licensed Master Social Worker (LMSW) license from the State of Michigan is required. Certification in Case Management (CCM) from the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) credentials from the American Case Management Association is preferred.
