Job description:
The Case Manager is an integral member of the patient-centered care team within the hospital or specialty setting, responsible for the collaborative practices involving identification, assessment, planning, facilitation, care coordination, evaluation, and advocacy of options and services to satisfy the health care needs of individuals and families. The primary objectives of the Case Manager include enhancing patient safety, ensuring quality care, and achieving cost-effective outcomes. The Case Manager provides comprehensive discharge planning services to enable a safe transition to the next level of care, addressing the requirements of patients who have encountered a critical event or diagnosis necessitating management strategies aimed at optimizing health outcomes throughout the care continuum.

Candidates must possess a Bachelor’s degree in Nursing or a Master’s degree in Social Work, alongside a minimum of three years of clinical experience. Strong verbal communication and written documentation skills are essential, along with exceptional customer service and interpersonal abilities that facilitate interactions with internal and external stakeholders at all organizational levels.
