Job description:
The Case Manager is an integral member of the patient-centered care team within the hospital or specialty setting, responsible for the collaborative practices involving identification, assessment, planning, facilitation, care coordination, evaluation, and advocacy of options and services to satisfy the health care needs of individuals and families. The primary objectives of the Case Manager include enhancing patient safety, ensuring quality care, and achieving cost-effective outcomes.
