Responsibilities
The Clare Bridge Program Assistant plays a pivotal role in assisting the Clare Bridge Program Manager or Coordinator in the planning and execution of person-centered dementia-friendly programs tailored to the unique needs of all residents within the dementia care community. In alignment with Brookdale’s commitment to Resident and Family Engagement, this role is dedicated to enhancing the quality of life for residents by providing meaningful and purposeful programming. The Assistant will recognize and honor individual preferences, habits, and routines, thereby encouraging resident independence.
Key Responsibilities Include:
- Supporting the implementation of individualized programming that caters to the unique interests and needs of residents across the 7 domains of well-being—purposeful, intellectual, social, spiritual, emotional, and physical—within a structured, dementia-friendly environment, including sensory-focused small group activities for residents unable to participate in larger gatherings.
- Assisting the Program Manager or Coordinator in reviewing individual service plans and completing required program documentation in accordance with Brookdale standards and state regulations.
- Ensuring compliance with all applicable regulations while maintaining alignment with Brookdale’s mission, policies, and standards in dementia care programming.
- Serving as a resource to associates regarding the significance of engagement and programming in enhancing residents’ quality of life.
- Coordinating and participating in community events, such as family socials and marketing open houses.
- Communicating positively with residents, families, and community professionals about the Brookdale dementia care program and providing updates on residents’ personal needs and achievements.
- Supporting the department’s quality improvement initiatives, including the Brookdale Excellence Standards Tool (BEST).
- Assisting in planning and executing outings or events outside the community, which may involve driving a community motor vehicle.
- Overseeing the volunteer program, including training and supervising volunteers.
- Performing personal care, housekeeping, and dietary duties as necessary to uphold service quality consistent with Clare Bridge Program standards.

Qualifications
Education and Experience
- High School Diploma or equivalent is required; an Associate’s Degree is preferred. A minimum of one year of experience working with older adults or individuals with dementia is preferred or an equivalent combination of education and experience.
Certifications, Licenses, and Special Requirements
- A valid driver’s license and a current three-year driving abstract, with no more than one moving violation, are required as a condition of employment.
Physical Demands and Working Conditions
- This position requires standing, walking, sitting, and the ability to use hands and fingers for handling or feeling items, as well as reaching with hands and arms. The ability to climb, balance, stoop, kneel, crouch, or crawl is also necessary.
- Requires effective communication skills, including talking and hearing, as well as the ability to lift up to 50 pounds.
- This role involves regular interaction with colleagues, residents, and vendors, with occasional weekend, evening, or night shifts as needed to ensure adequate coverage.
- Potential exposure to communicable diseases, latex, blood-borne pathogens, various drugs, chemicals, infections, or biological hazards.
- Occasional travel may be required.
