Job description:
Under general supervision, this role involves engaging one-on-one with participants from diverse backgrounds to assess their medical, behavioral, and social health needs. The position is responsible for collaborating with participants to formulate a personalized care plan and facilitating connections to community resources that enhance overall health. Key responsibilities include acquiring and documenting information related to participants’ barriers to health, acting as a liaison to help them navigate available community resources, and utilizing an evidence-based model to mitigate these barriers.

Education and outreach initiatives directed towards participants will be conducted through direct consultations and written materials. Compliance with established program policies and procedures is critical, including the utilization of designated pathways, case conferencing, and timely data collection and reporting. The incumbent will track and document activities and outcomes following established protocols and timelines.

Building trusting, authentic relationships with participants is essential for effective engagement. The role also involves providing information and social support concerning health behaviors, making referrals to external service agencies, and maintaining participant-related documentation in an electronic health record system. Outreach activities may take place in office, clinic, or community settings, including making outreach calls to current participants and potential referrals from Medicaid Health Plans due for clinical services.
