Job description:
Under general supervision, this role involves engaging one-on-one with participants from diverse backgrounds to assess their medical, behavioral, and social health and wellbeing needs. The position requires working collaboratively with participants to create individualized care plans and connecting them to community resources aimed at enhancing their health. Responsibilities include gathering and documenting information related to participants’ obstacles to achieving optimal health and serving as a liaison to identify and navigate community resources, utilizing an evidence-based model to address health and wellbeing barriers.
Essential duties include providing preliminary screenings for health and social service needs, identifying barriers, and outlining necessary pathways to ensure access to a comprehensive range of health and social services. The role entails conducting scheduled visits with participants, including home visits as necessary, to fulfill their specific needs. Acting as an advocate and liaison between participants and service providers is vital, as is assisting participants in understanding, accepting, and adhering to medical recommendations.
Additional responsibilities involve delivering educational outreach through in-person consultations and written materials, assessing eligibility for health insurance coverage, and offering enrollment support. Compliance with established program policies and procedures, including the use of pathways, case conferencing, and timely data collection and reporting, is required. The position requires tracking and documenting activities and outcomes in accordance with set procedures and timelines.
Building trust and authentic relationships with participants is crucial for effective engagement. The role includes providing information and social support regarding health behaviors, making appropriate referrals to external service provider agencies and internal programs, and preparing and maintaining participant-related documentation in electronic health records. Outreach activities may take place in office, clinic, and community settings, including calls to current participants and referrals from Medicaid Health Plans requiring clinical services.
Participation in ongoing training is essential for maintaining professional excellence within the Community Health Worker (CHW) field, alongside maintaining confidentiality regarding sensitive information.
Other duties may be assigned as needed, including front desk coverage at the same or different locations.
