Job description:
Responsible for delivering infusion medications and supplies to patients’ homes following preparation by the infusion pharmacy. This role includes picking up infusion supplies and equipment from patient residences as directed by the pharmacist. Additionally, the position involves the delivery, setup, removal, cleaning, storage, and maintenance of telehealth units in coordination with the equipment company.
Responsibilities:
- (60%) Review the home infusion admission booklet with patients, covering essential topics such as patient rights and responsibilities, consent, financial obligations, and points of contact for inquiries. Instruct patients on the proper storage of medications and supplies as directed by the pharmacist. Deliver and set up telehealth units, providing education to patients and caregivers.
- (30%) Pick up supplies and equipment, ensure the cleaning of infusion and telehealth equipment, and guarantee the appropriate storage and maintenance of these tools. Complete all required documentation for deliveries and promptly inform the pharmacist, supervisor, or manager of any issues or concerns.
- (10%) Verify that all items listed on the delivery ticket are present and coordinate the delivery and pickup with Home Care/Infusion designees and patients.
Other Duties and Responsibilities:
Participate in after-hours and weekend on-call rotations. Ensure that the company vehicle is maintained in a clean condition, monitor routine maintenance needs, and report any repair requirements. MyMichigan Health emphasizes technology, and employees must demonstrate proficiency in Microsoft® Windows. Participation in ongoing training opportunities offered by MyMichigan Health may be required. Additional duties may be assigned as needed.
Certifications and Licensures:
- Crisis Prevention Intervention (CPI): Required for employees hired before May 1, 2022, by May 1, 2023. For employees hired on or after this date, CPI Nonviolent Crisis Intervention certification is required within 90 days of hiring or transfer.
