Job description:

Unit: Second Avenue Dialysis (CA/CC & HHD)

Second Avenue Home Dialysis is an integral unit affiliated with an outpatient dialysis facility, specializing in the care of peritoneal and home dialysis patients. Our current patient census includes 25 peritoneal dialysis (PD) patients and 5 home hemodialysis (HHD) patients. The unit is staffed by a dedicated team of full-time nephrologists, complemented by 2 full-time home nurses and 1 home dialysis technician. We also benefit from multiple support staff members shared with the outpatient clinic, including front desk personnel and technical leaders responsible for machine maintenance, who may assist with operational duties within the clinic.

We actively manage care for 30 patients who are monitored through connected health platforms, including those from Baxter, NxStage, and Tablo systems. Patients receive training to conduct dialysis treatments at home within our clinic environment: PD training typically lasts 7-10 days, while HHD training spans 4-6 weeks. Our home team schedules monthly clinic visits for all home patients in coordination with an interdisciplinary team, ensuring comprehensive lab draws for PD patients. Additionally, our team provides patient education and facilitates referrals to transplant services as requested by patients.

The unit operates Monday through Friday, from 8:00 AM to 4:30 PM, with flexibility to accommodate clinic needs. Home nurses are also scheduled for after-hours on-call duties, including weekends. The home clinic generally remains closed on weekends and holidays, dependent on clinic needs, such as training sessions or urgent clinical care for patients.

General Summary: Under the supervision of nursing staff, the dialysis technician is responsible for monitoring and providing direct care to patients undergoing hemodialysis. This includes ongoing patient assessment and effective communication with other team members. The technician plays a vital role within the interdisciplinary team dedicated to the care of dialysis patients.

Education/Experience Required: A High School Diploma or G.E.D. equivalent is mandatory. Preferred experience includes roles such as Nursing Assistant (NA), Medical Assistant (MA), or other healthcare positions. Successful completion of the Greenfield Health Systems dialysis training program is required for continued employment.

Certifications/Licenses Required: Certification as a dialysis technician must be obtained and maintained within an 18-month timeframe, adhering to federal regulations.
