Job description:

This position is accountable for ensuring that all core responsibilities are executed in accordance with the company’s policies, procedures, and relevant standards, including Joint Commission standards (TJC), Licensing and Regulatory Affairs (LARA), HCBS guidelines, and the contractual requirements of Community Mental Health Agencies, along with other regulatory bodies. As a Direct Support Professional, you are expected to embody the goals and mission of the organization and act in alignment with our "I CARE" core values—Integrity, Compassion, Advocacy, Respect, and Excellence.

Responsibilities and Essential Functions:

  • Serve as a positive role model for residents at all times.
  • Follow and implement the person-centered plans and behavior plans as specified for each resident.
  • Create opportunities throughout the day to help residents achieve their goals, such as guiding group activities and encouraging participation.
  • Ensure the delivery of quality care in a safe and supportive environment, preserving the rights and dignity of all residents while fostering trust-based relationships.
  • Vigilantly protect residents from abuse and neglect, promptly reporting any incidents to the appropriate authorities.
  • Maintain the confidentiality of all residents.
  • Adhere to all house guidelines consistently.
  • Prepare meals for residents based on food specifications, dietary requirements, and applicable documentation.
  • Understand food supply and storage processes.
  • Provide transportation for residents to appointments as necessary, along with obtaining required documentation.
  • Assist residents in budgeting their funds as appropriate.
  • Complete mandated documentation before the end of each shift, including progress notes, daily support logs, incident reports, and resident inventories.
  • Oversee and assist residents with laundry tasks while also managing facility laundry as per assignment.
  • Coach residents in maintaining cleanliness in personal spaces, adhering to house guidelines.
  • Ensure the cleanliness of the home environment according to the assignment chart, maintaining readiness for inspection at all times.
  • Remain alert and accessible in homes requiring 24/7 care.
  • Complete mental health classroom training as required.
  • Administer medications as necessary.
  • Facilitate effective handover communication to the next shift.
  • Perform additional duties as assigned or requested.

Education and Qualifications:
