We are looking for compassionate and dependable Home Health Aides (Private Duty) to deliver personalized care to individuals in the comfort of their homes. In this pivotal role, you will assist clients with daily living activities, ensure their well-being, and provide much-needed companionship. Open Assignments:

• Lake Sheffield, Ohio – Monday to Thursday, 8 AM to 12 PM
• Middleburgh Heights – Wednesday to Friday, 1 PM to 6 PM
• Richmond Heights – 7 days a week, 9 AM to 1 PM

Job Responsibilities:

• Assist clients with daily tasks as outlined in their personalized care plan, which may include feeding, meal preparation, linen changes, light housekeeping, providing transportation to appointments, outdoor activities, and shopping
• Offer companionship and personal hygiene care, including bathing, grooming, oral care, dressing, undressing, toileting assistance, and measurements as needed
• Facilitate clients’ active and passive mobility needs as defined in the care plan, documenting any observed changes to the Director of Nursing
• Comply with the policies, procedures, and protocols of the agency
• Uphold HIPAA regulations to ensure the confidentiality and security of clients’ medical information

Job Requirements:

• High school diploma or GED
• At least one (1) year of documented experience in home care assistance or direct care within an assisted living or skilled nursing facility is preferred
• Preferred: Completion of a nursing assistant or certified home health aide program
• Current CPR certification and negative TB skin test or chest x-ray
• Valid driver’s license and auto insurance (proof of insurance required)
• Physical ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull; capacity to lift up to 50 pounds, depending on client mobility
• Self-motivated with the ability to work independently with minimal supervision
• Proficient in reading, writing, speaking, and understanding English, with effective communication skills
• Up-to-date vaccinations, including COVID-19, Influenza, and Hepatitis B
