Responsibilities:
- Assist clients with personal care and homemaking tasks in their residences
- Plan and prepare meals for clients requiring assistance
- Provide companionship and perform light housekeeping duties
- Adhere to care plans established under the guidance of a Registered Nurse

Core Responsibilities:
- Deliver personal care as needed
- Provide personal care services under the direction and supervision of a Registered Nurse
- Follow care plans accurately
- Assist with meal preparation and feeding
- Handle laundry and perform light housekeeping
- Adhere to infection control policies, including proper handwashing techniques
- Report any changes in clients’ conditions to the Registered Nurse
- Document care provided and maintain requisite client records
- Ensure personal safety and a safe environment for clients by observing infection control practices and reporting unsafe conditions to the Supervisor/Case Manager
- Demonstrate safe use of equipment and notify the Supervisor of any training needs
- Foster effective communication with the interdisciplinary team via verbal reports and participation in staff meetings
- Treat clients and their families with dignity and respect, ensuring confidentiality at all times
- Accept and fulfill assignments judiciously
- Participate in required in-service programs and identify any learning needs to the agency supervisor
- Perform other related duties as assigned
Additional Qualifications:
- Meet state training and certification requirements for home health aide services
- Hold an active, verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification or possess one year of verifiable supervised experience within the past three years
- Be at least 18 years old
- Exhibit competence in reading, writing, and following a written Plan of Care
- Demonstrate strong interpersonal skills and a genuine passion for providing care
- Hold a current driver’s license with a strong driving record, or possess reliable transportation
- Experience of 1+ years is highly preferred
- The role may require frequent standing, walking, stopping, bending, and occasional squatting or kneeling
- Ability to handle weights of 30 to 50 lbs as needed
