In your role as a Hospice Aide / Certified Nursing Assistant, your responsibilities will include:
– Reporting to the Administrator, Executive Director, or Senior Patient Care Manager while functioning under the supervision of the Registered Nurse and/or Clinical Supervisor.
– Assisting patients in maintaining personal hygiene and a safe living environment.
– Preparing and serving light meals and assisting with feeding as needed.
– Aiding patients in ambulation and the safe use of mobility aids, following training and supervision by a nurse.
– Performing light housekeeping duties, including trash removal and changing linens.
– Assisting with treatments as detailed in the aide’s plan of care and complying with state-specific regulations.
– Engaging patients in activities to promote mental alertness.
– Providing emotional and psychological support to patients and their families.
– Reporting any changes in the patient’s condition or home environment to the nursing team.
– Educating primary caregivers on basic care practices.
– Implementing infection control measures and adhering to standard precautions.
– Documenting visit details accurately and attending staff meetings.
– Participating in at least one Quality Improvement activity annually, as assigned.
– Completing all documentation promptly as outlined during orientation.
Candidate Requirements:
– A compassionate heart for hospice care, demonstrating kindness and empathy towards patients and their families.
– Comfort in providing care in various home settings.
– Ability to maintain composure and flexibility during crises.
– Certification as a Nursing Assistant (CNA).
– A minimum of six months of patient care experience in hospice, home health, ICU, geriatric, or oncology settings is strongly preferred.
– Additional preferred experience includes roles such as Hospital Staff RN, Med Surg, PACU, Float Pool, ER, Critical Care, Acute Care, Cardiac, Geriatric, Admissions, Wound Care, Telemetry, or Nursing Home care.
– A valid driver’s license and automobile insurance.
– Current CPR certification.
