Job description:
The Infection Prevention & Patient Safety Specialist plays a critical role in advancing the organization’s commitment to continuous quality and safety improvement. This position serves as a catalyst for change across various departments, leading efforts to educate the workforce and cultivate a culture of high reliability that prioritizes the elimination of preventable harm while fostering preventative care.
Key Responsibilities:
– Act as a resource in collaboration with management regarding standards set forth by the Joint Commission, RHC, CMS, State of Michigan, CDC, and other relevant regulatory bodies related to patient safety and infection prevention. Facilitate the development of strategies to ensure compliance with all regulatory requirements and assist in the creation and revision of policies and procedures as necessary.
– Conduct regular rounds in clinical care departments to evaluate and address issues related to quality, risk reduction, infection prevention, and the safety of patients and staff. Report back to department managers and leadership regarding any patient quality and safety concerns identified during these rounds.
– Collaborate with clinics to minimize risks to patients resulting from human or system errors through various root cause analysis methods. Provide proactive support through tracking and trending analysis to guide preventive care interventions, encompass infection prevention, disease monitoring, public health enhancement, immunization initiatives, safety assessments, and wellness updates.
– Proactively gather and report data pertaining to patient safety, quality, and regulatory initiatives as required. Oversee the investigative process for patient/staff safety and infection prevention variances as directed by Quality Leadership. Ensure thorough follow-up with all responsible parties, analyze trends in event reporting data, and present findings to leadership and relevant teams and committees as necessary. Assist departments in modifying processes, policies, and workflows based on insights from cause analysis. Monitor the progress of these changes at regular intervals to ensure their effectiveness and sustainability.
– Collaborate closely with the Director of Nursing to manage vaccine information and monitoring within ambulatory settings. Act as the Champion for Pediatric Wellness and Medicare Wellness Visits for MMG. Participate actively in the sharps and environment of care committee and serve as the BioMed interconnectivity champion, liaising with the BioMed team. Assist in the annual update of the Emergency Operations Plan and take responsibility for planning large drills biannually.
– Communicate relevant updates from the State of Michigan, RHC, CMS, CDC, and other appropriate bodies as necessary.
Additional Duties and Responsibilities in Patient Safety Quality:
– Facilitate teams that address identified opportunities for improvement based on monthly and yearly Ambulatory Quality data and input from Quality leadership.
– Coordinate the creation of evidence-based, standardized care practices and performance improvement activities to promote a high-quality, reliable care delivery system.
– Plan, prepare, and coordinate educational presentations for new employees and providers concerning quality, risk, and patient safety strategies.
– Ensure compliance with standards and requirements set by The Joint Commission (TJC), Centers for Medicare & Medicaid Services (CMS), RHC, CDC, and pertinent state quality assurance and performance improvement mandates by coordinating, organizing, and communicating necessary information to designated committees and leadership.
– Lead initiatives targeting high-priority opportunities as identified by leadership.
Certifications and Licensures:
– BLS: Basic Life Support (to be obtained within one month of hire or transfer; qualification source: AHA) – Essential.
– CIC: Certified Infection Control (to be obtained within 36 months of hire or transfer) – Essential.
Required Education:
– Associates degree is essential; RN licensure is preferred.
Experience, Training, and Skills:
– Three to five (3-5) years of clinical experience. Familiarity with variance reporting tools and processes along with a solid understanding of key safety and regulatory organizations and accreditation standards in healthcare (TJC, IHI, etc.) is required.
– Proven ability to monitor quality and take proactive measures to mitigate risk.
– Strong problem-solving skills, exceptional interpersonal and customer service capabilities, and effective oral and written communication skills are necessary. The ability to present information clearly in public forums is essential.
