Responsibilities:
– Participate in the planning and coordination of comprehensive patient care alongside care staff and physicians, adhering to individual nursing care plans.

Qualifications:
– Minimum of one year of experience as a Licensed Practical Nurse (LPN) in a home care setting.
– Current state license as an LPN.
– Graduate of an accredited practical nursing program.
– Current CPR certification.
– Flexibility to work varied schedules.
– Strong interpersonal skills.
– Valid driver’s license and reliable transportation.
– Capacity and willingness to travel locally.
