Job description:

We are seeking a compassionate Licensed Practical Nurse (LPN) to become a valued member of our expanding team. The ideal candidate will possess a positive, can-do attitude, exceptional interpersonal skills, and a valid state LPN license. In this role, you will provide essential medical support to our clients while ensuring a safe and nurturing environment.

Responsibilities:

  • Collaborate in the planning and coordination of comprehensive patient care alongside the Registered Nurse (RN) and the physician’s care plan, while adhering to the nursing care plan for each patient.
  • Accurately document and communicate the patient’s condition and care, identifying any changes in signs and symptoms.
  • Prepare detailed clinical progress notes and summaries.
  • Support patients in performing activities of daily living and educate them on appropriate self-care techniques.
  • Maintain a safe environment for patients at all times.
  • Assist physicians and Registered Nurses in managing complex nursing situations.
  • Adhere to universal precautions during all aspects of patient care.
  • Implement emergency procedures in response to any catastrophic events, incidents, or significant changes in a patient’s condition.

Qualifications:

  • Minimum of one year of experience as an LPN in a home care setting.
  • Current state LPN license.
  • Graduate from an accredited practical nursing program.
  • Proof of current CPR certification.
  • Flexibility to work varied schedules and travel locally.
  • Outstanding interpersonal communication skills.
  • Valid driver’s license and reliable means of transportation.
