Job description:
We are seeking a compassionate Licensed Practical Nurse (LPN) to join our expanding team. The ideal candidate will possess a proactive attitude, exceptional interpersonal skills, and a valid state LPN license. In this position, you will provide medical support to our clients while ensuring a safe and nurturing environment.

Responsibilities:

– Collaborate in the planning and coordination of comprehensive patient care alongside the Registered Nurse (RN) and physician, adhering to the established care plan for each patient.
– Accurately assess and document the patient’s condition and care, including any signs or symptoms that may indicate changes in their health status.
– Prepare clinical progress notes and summaries as required.
– Assist patients with activities of daily living while educating them on appropriate self-care techniques.
– Maintain a safe environment for all patients.
– Support the physician and RN in complex nursing situations as needed.
– Adhere to universal precautions when delivering any aspect of patient care.
– Implement emergency procedures during catastrophic events, incidents, such as accidents, injuries, or significant changes in the patient’s condition.

Qualifications:

– A minimum of one year of experience as an LPN in a home care setting.
– Current state license as an LPN.
– Graduate of an accredited practical nursing program.
– Proof of current CPR certification.
– Ability to work a flexible schedule.
– Excellent interpersonal skills.
– Valid driver’s license and reliable transportation.
– Willingness to travel locally as required.
