We are seeking a compassionate Licensed Practical Nurse (LPN) to become a valuable member of our expanding team. Responsibilities:

  • Collaborate with the Registered Nurse (RN) and physician to develop and implement comprehensive patient care plans, adhering to established nursing protocols for each individual.
  • Accurately document and report on the patient’s condition, noting any signs and symptoms that may indicate changes in health status.
  • Prepare clinical progress notes and summaries to maintain thorough documentation of patient care.
  • Assist patients with activities of daily living while educating them on appropriate self-care practices.
  • Ensure a safe environment for patients at all times.
  • Support the physician and Registered Nurse in managing complex nursing situations.
  • Adhere to universal precautions in all aspects of patient care.
  • Implement emergency procedures as necessary in response to catastrophic events, incidents, or significant changes in patient conditions.

Qualifications:

  • A minimum of one year of experience as an LPN in a home care setting.
  • Current state licensure as an LPN.
  • Graduate of an accredited practical nursing program.
  • Valid certification in CPR.
  • Flexibility to work various schedules as needed.
  • Outstanding interpersonal skills.
  • Valid driver’s license and reliable transportation for local travel.
